London Borough of Lambeth
Lambeth is a London borough in South London, England, which forms part of Inner London. Its name was recorded in 1062 as Lambehitha ("landing place for lambs") and in 1255 as Lambeth. The geographical centre of London is at Frazier Street near Lambeth North tube station, though nearby Charing Cross on the other side of the Thames in the City of Westminster is traditionally considered the centre of London.
Social Prescribing in Lambeth
Other Social Prescribing initiatives include:
1. Safe and Independent Living (SAIL) and SAILplus schemes delivered by Age UK Lambeth. Service receives referrals from Lambeth GPs, self referrals, local acute hospitals and community health and social care teams. Following contact with individual, SAIL will link them directly to appropriate service e.g. financial advice, handyperson, befriending, social groups, dementia support etc. For those individuals contacted who present with more complex issues, SAILplus will discuss with individual concerns and work with the individual to resolve.
2. Project Smith is a localised community development programme with two workstreams - small grants for improving health and wellbeing in a local area; and Community Connectors - interested people who connect others to local support, information, groups, activities. Community Connectors receive Royal Society of Public Health Supporting Behaviour Change training, and meet monthly to share and learn from each other.
3. Primary Care Navigators in GP surgeries and local pharmacies.
