Social Prescribing in Harrow

Social Prescribing in Harrow started in June 2017. Accessed via GP, health professional or self-referral, via a simple form that then triggers a holistic screening of the patient that leads to the offer of a prescription, bespoke to the patients needs of a plan to achieve better self-management of their health and overcoming the determinants of ill health.

Services are offered within three categories, as listed below:


  • Support in Mental Cognition
  • Support with managing diabetes
  • Support with managing hypertension
  • Support with falls
  • Support with healthy eating


  • Housing  
  • Welfare benefits
  • Money /debt
  • Employment
  • Well-being (i.e health and social care not covered by the Care Act SWISH contract)
  • Family & personal matters (counselling, bereavement, abuse, relationships, older people, carers)
